The white Mini DisplayPort 1.2 to HDMI 4K Audio/Video Converter from StarTech enables you to connect a Mini DisplayPort-equipped device to an HDMI display. This adapter is compatible with Mini DisplayPort 1.2 and HDMI 4K at 30 Hz and as a result can support video resolutions up to Ultra HD (4K) as well as digital audio up to 7.1 channel surround sound.
With support for Ultra HD (4K) the adapter ensures that you can convert Mini DisplayPort to HDMI while maintaining a picture quality four times the resolution of high-definition 1080p. The adapter is also backwards compatible with previous DisplayPort revisions 1.0/1.1 with support for resolutions up to 1080p. For a hassle-free setup the Mini DisplayPort to HDMI adapter allows for plug-and-play installation without any additional software or drivers. With a small footprint and lightweight design, the adapter maximizes portability. To ensure compatibility with any Mini DisplayPort output, the adapter offers an active conversion. Active video conversion is required to maintain 4K resolutions when converting Mini DisplayPort to HDMI. It is also ideal for outputting 1080p resolutions by ensuring compatibility with graphics cards that are not capable of outputting multi-mode DP++ signals.

Use this active adapter to connect a third monitor to a MacBook Pro mid 2015. Connect to first TB port and use DVI to TB and HDMI to HDMI for other two monitors.

My five star rating is for the fantastic support I received in trying to get the adapter to work with my Surface Pro 4. Unfortunately it still does not work, but not for lack of patience and a very professional effort at troubleshooting. Microsoft seems to have an issue with the Mini-display port on that machine they have yet to fix.
